Bringing Primary Care Into Residential Communities
Our mission is to expand access to trusted, high-quality primary care by bringing personalized healthcare directly to people living in boarding homes, group homes, and residential care settings, as well as individual patients who benefit from care at home.
We focus on building meaningful relationships, supporting continuity of care, and working collaboratively with patients, families, caregivers, pharmacists, and residential care teams when appropriate.

Katherine Barnes, FNP
Founder & Chief Provider
RN, BSN, MSN, FNP-C

- 15+ years of medical and clinical experience
- Comprehensive primary care background
- Committed to home-based and residential care
Katherine became a provider with a purpose: to advocate for those who too often go unseen and unheard in healthcare, and to make every individual feel valued and respected.
Over more than 15 years in the medical field, her experience has spanned critical care in the CCU, the ER, case management, long-term care, and geriatrics. She holds a Bachelor's degree from West Virginia University and a Master's degree from Walden University, and is board certified by the American Association of Nurse Practitioners as a Family Nurse Practitioner.
Today, she specializes in comprehensive primary care for individuals who benefit from receiving care at home and those living in group and residential care settings. Her practice is open and affirming to all, with a special focus on people who have historically been marginalized or underserved — including those from minority backgrounds and the LGBTQIA+ community.
